Midwest airlines I think would work from BMI to MKE to DFW but you would have to check if the aircraft is large enough for an adult from BMI to MKE. They do adults and you have to meet the temp requirements but they are pretty flexible if the airlines are big enough. Some of the other airlines you have to check with if the dog is more than 100# with the crate.
There would be 3 criteria to check. Size of the airplane to carry a 400 or 500 crate, temperature requirements, and total weight requirement.
Check Delta, American and Northwest also. They all do the routing.

I've been told about restrictions into D/FW during the summer. I don't know that for a fact but I've heard from reliable resources. Calling the airlines would probably be your best bet.

This site has all airlines listed wtih their policies regarding shipping dogs.
You could use Continental (no heat restrictions), but the dog will have to go through Houston.
