submit CNM/EIC result to OFA

I looked on the OFA website and guess I'm not finding it or it's not there.

How do you submit your CNM (or EIC) result for your dog to be posted on the OFA database? (I know there's a fee)

I have a few dog's with results that i have received a while back & just want to get them posted in one place.

With the EIC results I received instructions on submitting to ofa. I was told to send a copy of the results to ofa with a check for $ 15.00. I just received confirmation from ofa that the info is now in their data base.

Check on your EIC results page I think you will find the info there.

This is the form you need to fill out (for each dog) and submit to OFA. This form can be used for any DNA-based test, EIC, CNM, PRA, whatever. OFA has certain restrictions on which labs they will accept resullts from. Below is the link that lists out all the DNA tests that OFA will certify, and which laboratories they will accept results from for each type of test.
