I've raised 4 puppies in last 2 years and this one has me baffled.

12wk old can sleep 10pm to 6am without a problem, but can't go from 7:30-11am without peeing in crate. At lunch, she will go outside and potty, but pee's again in crate from 12:30 to 3:30 when daughter gets home from school.

See is fed in morning, but water is very limited. (this morning none)

The amount in crate is very small but still get's her wet. The crate is small also. She can not stand at full height.

Do I go to even smaller crate? Thanks Warren, forgot to mention $144 later no UTI everything normal.

It's like she gets out and starts hunting worms or playing and forgets to take care of business

Baffled in Arkansas....at least about the puppy, but not about the coach.

I haven't personally had this problem(Newbie) but my girlfriend brought a rescue dog home from the shelter at 10 weeks old. Her puppy did the same thing till she was 4 months. She pretty well knew when she was inside the house but out of her crate that she needed to do her business outside. She started cutting the potty training pads in half and sticking one in there with Jesse when she would leave. She did it for a week or so and forgot to put one in one morning but left the old one laying right next to it. When Abby got home she let Jesse out, she ran right to the pad and did her business on it. Abby moved the pad further from the kennel until Jesse got it under control.

Don't know if that will help at all. Have you tried making the kennel as dark as possible during the day?
