I've started "hold" with my girl. How long do you make her hold and when do you know it's time to start the fetch portion?

Generally to speak, hold is not set on a time schedule. I am sure that most people teach hold until pup will walk at heel with the bumper in his or her mouth without dropping and hold the bumper with distractions and taps.
